Application to Adopt our Dogs

The Adoption Process

Finding a loving and caring forever home for our rescued dogs is our top priority as well as their safety and well-being. Our team of volunteers & professionals provides the necessary care and attention to help these dogs find their forever homes.  We look forward and encourage you to start the adoption process below with us!

Adoption Application

Did you see one of our rescues that looks like a great addition to your family? That's great news for the dog and for us!  Fill out the adoption application and we'll get back to you as soon as possible.

Do you have a fenced yard?
Have you owned a dog before?
Do you currently have any dogs?
Do we have pemission to call your vet
Will you allow a rescue agent to inspect your home & yard prior to placement?
Are you willing & able to take obedience classes?

Should this dog not work out in this home, it must be returned to Furgotten Friends Dog Rescue. If it is over 1 month, an additional fee could be charged for a wellness check from the agency's veterinary of choice.        

 

I/WE CERTIFY THT ALL THE INFORMATION ABOVE IS ACCURATE AND TRUE. I/WE AGREE TO ACCEPT THE DECISION OF FURGOTTEN FRIENDS DOG RESCUE REGARDING MY/OUR SUITABILITY FOR ADOPTING A DOG.

I/WE UNDERSTAND THAT THEIR DECISION IS FINAL AND NOT OPEN TO ARBITRATION. IF NOT APPROVED, NO EXPLANATION OR JUSTIFICATION WILL BE MADE AVAILABLE.
